The following forms may be turned in at your nearest branch, emailed or faxed. The email to member services is memberservices@smartcu.org and the fax number is 713-622-6860. All documentation sent to member services via email requires PDF format. Member services handles requests in the order they are received during regular business hours. Requests must be received from the applicable member and documentation received with incorrect, incomplete information, or missing signatures will be automatically be rejected.

Wire authorizations can be requested at any of our branches. The Wire Authorization Form requires a wet signature along with a copy of valid driver's license. We do not take wire requests via phone or fax.
